    a "log book"

    Well since it is quite obviously a form, not a book, and since it has written in large letters on it

    "VC5 form", I thought maybe it would be apppropriate to call it...oh, I dunno...maybe like a VC5 form or something?

    Log sheet would be better for Monty.

    Or if he had learnt to drive when he was at a proper age, he would know that they did indeed used to be put together like a booklet with different pages.

    No, VC5 really would be fine.That way, when I'm told I need my VC5

    I can look at the piece of paper with "VC5" written on it and think ah yes, there's my VC5.

    Isn't it V5C? So in actual fact you would still be struggling thinking if there was another form.

    I agree. Even though many years have passed since I first discovered that the call a piece of paper

    a book, it is silly to call a form a book imo.
