The race is always neutralised as they get away from the start town, or there'd be a massive pileup straight from the start.

The race is sometimes neutralised by the riders (e.g. at a feed zone, it's considered poor etiquette to attack there) just by common practice or consent amongst the workers of the peloton (e.g. if a stretch of road is considered too dangerous to race on for some reason), sometimes as an industrial dispute regarding working conditions.
